What you'll do
- Assess youth in crisis alongside a nurse
- complete intake and diagnostic assessments
- guide care flow and coordinate internal and external referrals
- document according to agency and state standards
- collaborate with community partners
- provide culturally responsive, strengths-based support to youth and families
- support data collection and provide virtual crisis support to other locations as needed.
