What you'll do

• Complete MDS assessments, Care Area Assessments, and care plans
• coordinate resident care with physicians, families, payers, and facility staff
• communicate regulatory risks and clinical trends to leadership
• collaborate with therapy staff on assessment timing and RUG scores
• submit MDS data and follow up on validation reports
• facilitate care management and discharge planning
• educate interdisciplinary team members on the RAI process
• analyze quality data
• complete Triple Check
• participate in facility committees
• maintain required documentation and follow the company code of conduct.
