Program Specialist — Community Health Programs

The Program Specialist — Community Health Programs role is onsite in New York, NY. Pay is $20 per hour. BronxWorks supports individuals and families through programs serving children, youth, families, seniors, and people experiencing homelessness; its Community Health Programs provide wellness, healthy-lifestyle support, and civic engagement programs to the Bronx. The specialist provides direct client services and supports service plans through home visits, referrals, advocacy, documentation, and follow-up. The role helps clients access services and track progress while also providing clerical and program support.

What you'll do

• Attend home visits with a case planner and independently
• report concerns about child safety and well-being
• accompany parents to appointments and programs
• support service plans and benefits access
• obtain school and medical records
• follow up on service-plan tasks
• document work in program forms and databases
• provide intake, engagement, assessment, referral, advocacy, counseling, exit summaries, and follow-up
• monitor client progress
• provide clerical support and assist with special projects.
