Registered Nurse Case Manager and Recertification Nurse

The Registered Nurse Case Manager and Recertification Nurse role is remote, serving clients across Northwest Ohio with travel to client homes and service locations. Compensation is $36–$40 per hour plus mileage reimbursement. Home Care Network is a nurse-owned, locally led home health agency focused on patient care and wellbeing. The nurse coordinates and reassesses home health services, provides direct nursing care, and supports care teams through clinical oversight and documentation. The role helps confirm ongoing medical necessity, maintain compliant care plans, and promote positive outcomes for clients and their families.

What you'll do

• Manage cases, starts of care, admissions, recertifications, transitional care, resumptions of care, clinical reviews, and assessments
• perform occasional skilled nursing visits and services
• assess clients and initiate appropriate care actions
• supervise RN, LPN, home health aide, and personal care aide teams
• document and communicate changes in patient status
• coordinate with physicians, agency staff, caregivers, clients, and families
• travel to assigned clients within the agreed service radius.
