The director leads program development and standardizes dementia-focused care practices, educates hospice teams and families, and helps integrate meaningful sensory and non-pharmacological interventions. The role also builds relationships with memory care communities and other healthcare partners to improve support for patients and families.

What you'll do

• Lead the Encore Program

• collaborate with Clinical Directors and Patient Relations Managers to introduce the program to healthcare partners.
