The Admissions Registered Nurse (RN) position is an onsite, field-based role serving patients in Colorado Springs, Colorado. Pay is $110–$125 per hour. Frontier Home Health’s admissions team supports the agency’s growth and helps extend home health services in the Colorado Springs community. The nurse completes start-of-care and other admission assessments, develops care plans, documents OASIS assessments, coordinates physician orders and interdisciplinary services, and provides skilled nursing care. The role supports safe transitions into home health and ensures care and documentation meet patient, regulatory, and agency requirements.

What you'll do

• Complete start-of-care, admission, and resumption-of-care assessments
• develop and implement individualized plans of care
• complete OASIS assessments and clinical documentation
• coordinate with physicians, therapists, and interdisciplinary team members
• communicate condition changes and verify orders
• administer skilled nursing care, medications, and treatments
• identify additional service and equipment needs
• support discharge planning, case conferences, and quality improvement.

Why this role
