The Community-Based Qualified Behavioral Health Specialist role is onsite in Lorain, OH, with community-based and telehealth services, and pays $24–$30 per hour. ACE Wellness Center’s behavioral health program provides individualized care planning and management to youth, families, and adults in schools, homes, and communities. The specialist supports client progress through coordinated, client-centered services, working with a licensed clinician to provide Community Psychiatric Treatment Services and Therapeutic Behavioral Services as needed.

What you'll do

• Interact with clients and families to track progress and satisfaction
• record case information and complete required paperwork
• assess and address motivational and psychosocial issues
• coordinate care and information sharing
• create service plans and assist with case closures
• coordinate with a licensed clinician to provide services as needed.
