Residential Assistant

What you'll do

• Assist residential leadership and case managers with case coordination and follow-up
• monitor assigned needs, appointments, referrals, and services
• gather documentation
• communicate client needs to appropriate team members
• support transitions, admissions, discharges, and continuity of care
• complete records and tracking tasks
• assist with meetings, audits, program operations, transportation scheduling, and supervision of Peer Specialists as assigned.
