The Crisis Care Coordinator — REACH Youth Crisis Team role is onsite in Milwaukee, WI. Pay is $27 per hour. Lutheran Social Services of Wisconsin and Upper Michigan’s REACH program supports youth in psychiatric crisis and works to prevent out-of-home placements by connecting youth with community resources. The coordinator provides trauma-informed, person-centered care coordination to help youth and families navigate crises and access support. Assessments, individualized service plans, crisis intervention, and collaboration with families and providers contribute to stability and community-based care.
