Hospice Social Worker (LICSW)

The Hospice Social Worker (LICSW) role is field-based, serving patients in their homes and facilities in Bedford, New Hampshire, and across the state. Compensation is $33.75–$37 per hour. LilyCare Hospice provides community-based end-of-life care to patients and families. As a member of the Interdisciplinary Team, the social worker provides psychosocial support and helps patients and families navigate emotional, practical, and financial aspects of end-of-life care. The role coordinates resources and care planning with the team and supports families through bereavement follow-up.

What you'll do

• Complete psychosocial assessments and contribute to plans of care
• provide counseling and emotional support to patients, families, and caregivers
• evaluate insurance coverage and financial needs and connect families with resources
• participate in interdisciplinary team meetings
• make referrals and coordinate services
• support bereavement follow-up
• document visits and services
• assist with volunteer and staff training and support groups as needed
• participate in quality improvement activities.
