The Medical Assistant (Health Navigator) — Senior Health role is onsite in Phoenix, AZ, with a Monday–Friday schedule from 8:00 am to 5:00 pm, 40 hours per week. Compensation is $23 per hour. The role supports a primary care team focused on members aged 65 and older at One Medical. The Health Navigator builds supportive relationships with patients and families and helps the care team address preventive, chronic, and acute care needs. The role combines clinical support, care coordination, patient education, and practice operations to improve access and the in-office patient experience.

What you'll do

• Room patients
• capture health information
• take vital signs
• administer point-of-care testing and age- and condition-appropriate screening assessments
• coordinate post-visit appointments and referrals
• share information with internal and external care teams
• collaborate with providers to monitor preventive care
• answer calls and emails and assist with front-desk inquiries
• maintain clinical space
• educate patients on chronic disease management and use motivational interviewing-based action planning
• provide anticipatory preventive guidance to families with children.
