Home Health Aide

Job overview

The Home Health Aide role is onsite in East Boston, MA, providing in-home assistance and potentially working in other supervised settings such as the PACE Center. Compensation is $22.00–$31.00 per hour, based on experience. The role supports Neighborhood PACE participants. The aide follows participant care plans and provides assistance with daily living and instrumental activities, while documenting care and reporting changes to the team. The role contributes to participant safety and continuity of care through personal assistance, appointment accompaniment, and simple treatments under nursing direction.

What you'll do

• Follow ADL/IADL care plans and document care
• check health aide voicemail at shift start and every two hours
• report abnormal findings, status changes, or care refusals
• assist with assigned daily living activities
• accompany participants to appointments as assigned
• perform simple treatments under nursing direction, including simple catheter care and skin assessments during personal care.

Why this role
