What you'll do

• Oversee referrals, intake, service initiation, staff assignments, scheduling, and service coordination
• review service plans and authorizations
• recruit, onboard, supervise, coach, and retain Direct Care Workers
• monitor service utilization, documentation, and staffing
• support audits and incident follow-up
• develop referral relationships and a program growth plan
• report program performance to leadership. Local travel and occasional response outside standard business hours are required.
