What you'll do

• Verify the plan of care with the RN before each visit
• collaborate with physicians and the RN and promptly report changes in patient condition
• teach patients and caregivers self-care techniques
• check vital signs
• review medications and complete medication reconciliation at each visit
• perform physician-ordered specialized procedures
• prepare treatment equipment and materials using aseptic technique
• complete documentation accurately and on time. Requires a current state-specific LPN license, CPR certification, at least one year of LPN or LVN experience, a valid driver’s license, reliable transportation and liability insurance.
