The Social Worker (LCSW) – Hospital role is onsite in Lawrenceville, GA. Pay is $1,500–$1,920 per week. The 13-week assignment is a day shift, 8:00 AM–4:30 PM, Monday–Friday, for 40 hours per week, supporting patients and families in an acute hospital setting. The role provides psychosocial support, care coordination, and discharge planning to help patients access appropriate post-acute services and community resources. It contributes to continuity of care through assessment, counseling, advocacy, collaboration, and timely documentation.

What you'll do

• Assess psychosocial needs and develop care plans
• provide counseling, emotional support, and crisis intervention
• coordinate discharge planning and post-acute services
• connect patients and families with community resources
• collaborate with physicians, nurses, case managers, and multidisciplinary teams
• assist with insurance, placement, transportation, and social service needs
• document patient care
• and advocate for patients.
