Behavioral Health Care Coordinator

Job overview

The Behavioral Health Care Coordinator role is an onsite position in Jasper, FL. Pay is $16–$18 per hour. The role is in the Managed Care department at North Florida Pediatrics and helps rural patients navigate mental health and substance use disorder treatment while connecting behavioral health facilities with primary care providers. The coordinator develops care plans, supports patient follow-up, and connects patients and providers with appropriate services. The work helps integrate behavioral health and primary care and supports continuity of care, including follow-up after emergency visits or crisis interventions.

What you'll do

• Develop personalized care plans
• coordinate monthly telephonic or telehealth check-ins
• help patients find clinics and behavioral health services
• connect primary care providers with psychiatric consultants and community support teams
• guide patients through virtual consultations and therapy
• maintain patient tracking and compliance documentation
• follow up after emergency room visits or crisis interventions.
