What you'll do

• Collaborate with the multidisciplinary team to develop and coordinate the plan of care.
• Communicate with patients, families, and external resources.
• Evaluate and document patient response to the plan of care.
• Manage a caseload of patients and identify necessary resources.
• Participate in performance improvement teams and data analysis.
