What you'll do

• Serve as attending physician for patients in assigned skilled nursing facilities
• collaborate with NPs and PAs to manage daily patient care and provide clinical supervision
• diagnose and treat acute and chronic conditions through patient-centered primary care
• participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
