The Bilingual Community Doula Navigator role is an onsite, full-time position based in Monroe, LA, with a starting rate of $18–$23 per hour. Malama Health uses technology and care navigation to support pregnant and postpartum mothers and improve maternal health outcomes. The navigator provides outreach, assessments, care coordination, and referrals for members while building relationships with local providers and community organizations. The role also documents services, supports patient learning, and raises awareness of Malama’s services in assigned communities.

What you'll do

• Conduct patient outreach, check-ins, and assessments by phone, text, video, or in person
• assist with appointments and social service applications
• coordinate referrals with health plans, providers, and care teams
• use motivational interviewing, trauma-informed care, and harm reduction
• document services in care management portals
• conduct community outreach and support events
• and travel regularly across assigned counties.
