What you'll do
- Provide evidence-based primary and preventive care, manage chronic and acute conditions including obesity, diabetes, hypertension, and dyslipidemia, and develop individualized treatment plans. Coordinate longitudinal care and referrals with a multidisciplinary team, provide clinical guidance, participate in quality improvement and population health initiatives, review clinical data, and maintain accurate medical documentation. The role averages approximately 16 patients per day and requires supervision of and collaboration with non-physician providers

Why this role
Requires an MD or DO and current or eligible unrestricted California medical licensure. Board certification in Family Medicine or Internal Medicine is preferred; medical Spanish fluency is strongly preferred.
