What you'll do

• Hold client sessions weekly, bi-weekly, or monthly depending on service
• communicate with clients, mentors, companions, family members, providers, and financial guarantors
• develop and revise reintegration plans
• provide phone support
• supervise and support mentors and companions
• pre-approve related reimbursements
• document interactions
• manage toxicology-screen information and medication management with mentors and companions
• transport clients if needed.
