What you'll do

• Provide admission, care management, and follow-up visits
• assess, diagnose, plan, implement, and evaluate care
• develop and update plans of care
• provide physical assessments, medication management, wound care, IV therapy, tube feeding, equipment setup and training, patient education, and comfort care
• coordinate with clinicians
• travel to patients’ homes or facilities
• update physicians
• supervise LPNs and aides as needed
• teach patients and families
• comply with HIPAA and regulations
• document clinical notes.
