What you'll do

• Assess patients in their homes
• lead and collaborate with the care team
• perform admission, readmission, and discharge functions
• conduct field and supervisory visits
• develop and monitor care plans
• coordinate referrals and physician orders
• complete OASIS and clinical documentation
• educate patients, caregivers, and healthcare workers
• supervise assigned personnel
• participate in QAPI and the agency’s 24-hour on-call service.
