The Pharmacist, Home Infusion role is an onsite position in Syracuse, NY. Pay is $124,800–$145,600 per year. KabaFusion provides home infusion pharmacy services through a national network of pharmacies and nursing offices. The pharmacist coordinates safe, appropriate pharmacy services for patients receiving care in alternate settings, assessing therapy, dispensing medications and supplies, documenting progress, educating patients and caregivers, and coordinating with the care team.

What you'll do

• Assess home-care suitability and prescribed therapy
• develop patient-specific pharmacy plans
• recommend and write appropriate orders
• assess laboratory results and perform pharmacokinetic dosing
• maintain medication profiles
• enter and dispense prescriptions
• troubleshoot medication pumps
• document communications and patient progress
• support compliant compounding
• participate in on-call processes
• oversee and train pharmacy support personnel.
