What you'll do

• Assess patients’ physical, functional, psychosocial, and environmental needs
• determine eligibility and service frequency
• develop, implement, document, and revise plans of care
• educate patients and families
• provide preventive and rehabilitative nursing procedures
• apply infection-control precautions
• direct home health aides and LPN/LVNs
• document progress
• communicate changes to physicians and care teams
• monitor assigned cases for payer compliance.
