• Build relationships with hospitals, rehabilitation centers, senior communities, physicians, social workers, and other healthcare professionals.
• Identify referral sources, represent the agency at community and networking events, promote services, assist with social media and promotional materials, track outreach and referral results, and collaborate with agency leadership.
