Registered Nurse (RN) Case Manager – Hospice

The Registered Nurse (RN) Case Manager – Hospice role is field-based in Onalaska, WI, with a Monday–Friday schedule from 8:30 a.m. to 5:00 p.m. Pay is $75,000–$90,000 per year. Moments Hospice focuses on comfort, dignity, and compassionate care for patients and families at the end of life.

What you'll do

• Complete comprehensive assessments of patients’ physical, emotional, and psychosocial needs
• develop and update individualized care plans with patients and families
• provide skilled nursing care, pain and symptom management, medication administration, and comfort measures
• educate patients and caregivers
• coordinate care and serve as a liaison
• and participate in interdisciplinary group meetings.
