Hospice Registered Nurse (RN) Case Manager

Working with the Clinical Director, the RN Case Manager leads a care team and coordinates patients’ home hospice care. The position develops and implements care plans, provides clinical assessments and medication management, educates families, and communicates patient progress to the clinical manager, physician, and care team.

What you'll do

• Coordinate and oversee patients’ home care
• collaborate with care teams to develop and implement care plans
• educate families about patient condition, needs, and care
• perform head-to-toe assessments
• check vital signs and manage medications
• administer and maintain catheters
• complete documentation accurately and on time
• supervise home health aides
• communicate patient progress to the clinical manager, physician, and care team.

Why this role

Requires a current Registered Nurse license, driver’s license, and CPR certification; two years of clinical RN experience and one year of full-time hospice experience are preferred.
