What you'll do

• Assess needs and provide support by phone or visit
• implement care focused on comfort, safety, and patient and family goals
• gather physical, psychological, social, and spiritual information for care planning
• collaborate on level-of-care decisions
• arrange medications and supplies
• support patients and families at time of death
• document clinical visits and care changes
• triage referral calls and complete admissions as needed
• report shift activity to the hospice team.
