What you'll do

• Provide one-on-one care in a personal setting
• assist with bathing, dressing, grooming, toileting, hygiene, and feeding
• support ambulation, transfers, and mobility
• assist with meal preparation, hydration, and nutrition
• maintain a safe, clean, comfortable home environment
• follow care plans with home health nurses
• observe patient condition and report changes to the supervising nurse
• communicate with patients, families, and care team members.
