What you'll do

• Coordinate appointments and communication with clients’ clinical and medical providers
• provide transportation to appointments or essential services as needed
• support treatment-plan adherence and monitor well-being
• assist with medication counts and adherence in coordination with the clinical team
• document interactions and observations
• provide emotional support and assist with household and lifestyle tasks.
