Community Health Worker

Job overview

The Community Health Worker role is onsite at New Hope Missionary Baptist Church in Wayne, MI. Compensation is $18–$20 per hour. The worker is part of the Wayne FQHC Program’s interdisciplinary clinical care team.

What you'll do

• Conduct patient outreach and provide education on preventive screenings and chronic disease management
• address HEDIS care gaps and connect high emergency-department utilizers with primary care providers
• conduct social determinants of health screenings and connect patients to community resources
• coordinate referrals and scheduling
• document outreach and prepare monthly reports
• participate in community outreach events and support site visits
• maintain HIPAA compliance and support patient panel management.

Why this role
