Hospice Medical Social Worker

• Assess patients’ psychosocial status and communicate findings to registered nurses
• conduct social evaluations and plan interventions
• maintain clinical records
• provide information and referrals to patients, families, caregivers, and community agencies
• serve as a liaison with community agencies
• collaborate with organization personnel
• participate in education, care planning, case conferences, and discharge planning.
