What you'll do

• Oversee completion of MDS, CAAs, and care plans in accordance with regulations
• coordinate resident care and communicate with residents, families, clinicians, and payers
• monitor regulatory risks and clinical trends
• collaborate with therapy staff on assessment reasons and dates
• submit MDS data and follow up on validation reports
• facilitate care management and discharge transitions
• educate interdisciplinary team members on RAI processes
• analyze quality data
• complete utilization review and Triple Check.
