What you'll do

• Coordinate medical, dental, and psychiatric appointments
• transport residents to appointments and outings
• communicate with physicians, medical offices, families, and service providers
• support residents with daily living skills and psychiatric symptoms as needed
• help residents access community and healthcare services
• monitor and document progress and appointment outcomes
• maintain accurate documentation
• work with the treatment team and facility staff.
