What you'll do

• Coordinate safe, timely, equitable, culturally sensitive, client-centered care
• manage case assignments and assess recovery barriers
• contribute to service and mental health treatment plans
• collaborate with counselors, psychiatrists, and nurse practitioners
• provide follow-up, referrals, and linkage to higher levels of care when medically necessary
• support daily-living goals
• complete billing and documentation
• protect confidential health information
• coordinate with community agencies and advocate for client needs.
