What you'll do

• Assess spiritual needs of patients and families or caregivers and ensure appropriate services are provided
• participate in comprehensive assessments and individualized care planning
• provide direct spiritual care
• liaise with community chaplains and spiritual counselors
• document services and communications
• attend patient-care conferences and interdisciplinary meetings
• provide spiritual-care consultation and education
• recruit community spiritual-care volunteers
• provide requested funeral or memorial services and plan periodic memorial services
• participate in quality improvement activities.
