Hospice Registered Nurse (PRN/Contingent)

The Hospice Registered Nurse (PRN/Contingent) role is an onsite field position serving Monroe County and surrounding areas from AdvisaCare’s Michigan hospice operation. Compensation is not specified. AdvisaCare provides hospice care intended to support comfort and dignity for patients nearing the end of life. The nurse assesses patients’ needs, implements and revises individualized care plans, educates and supports patients and families, supervises paraprofessionals, documents care, and participates in interdisciplinary meetings and quality improvement.

What you'll do

• Assess patients’ physical, functional, psychosocial, and environmental needs
• implement and revise individualized plans of care
• educate patients and families about disease processes, self-care, and end-of-life care
• supervise paraprofessionals
• prepare clinical and progress notes
• participate in interdisciplinary group conferences and performance improvement
• communicate care information and maintain client and family satisfaction.
