What you'll do

• Provide acute care, hospital and ER follow-ups, chronic disease management, preventive care, and Medicare Annual Wellness Visits
• evaluate, diagnose, and treat adult and geriatric patients with multiple chronic conditions
• prescribe medications, order imaging, review results, and coordinate referrals and follow-up care.
