What you'll do

• see approximately 12–15 patients daily with 20–40-minute visits
• collaborate with an integrated team that includes nurse practitioners or physician assistants, medical assistants, social workers, lab and pharmacy support, and referral coordination
• complete onboarding and training in value-based care, documentation, EMR use, and care-model workflows.
