Peer Support Counselor – Nursing Facility Transition and Outreach

The Peer Support Counselor – Nursing Facility Transition and Outreach role serves the Upper Eastern Shore of Maryland, including Kent, Queen Anne’s, Caroline, Talbot, and Cecil counties. It is approximately 15 hours per week at $18 per hour. Accessible Resources for Independence is a nonprofit Center for Independent Living that promotes independence and equal access for people with disabilities. The counselor helps nursing facility residents explore community living and transition options by building relationships, identifying useful services, developing goals with residents, and connecting them with community resources and partners.

What you'll do

• Visit nursing facilities monthly
• build relationships with residents, families or representatives, ombudsmen, and facility staff
• provide information about community living and transition processes
• identify services for people moving back to community life
• develop goals with residents or representatives
• attend relevant meetings and events
• deliver resource materials
• maintain accurate case notes and records
• travel throughout the listed counties.
