Hospice Registered Nurse (RN), On-Call

What you'll do

• Conduct initial and ongoing comprehensive assessments
• apply hospice admission and recertification criteria
• develop, implement, and document plans of care
• educate patients and families about disease processes, self-care, end-of-life care, and nutrition
• determine service scope and frequency
• perform preventive and rehabilitative nursing procedures
• apply infection-control precautions
• direct hospice aides and LPN/LVNs
• prepare clinical and progress notes
• communicate changes and outcomes to physicians and other personnel
• monitor cases for payer compliance
• perform on-call responsibilities as assigned.

Why this role

Requires graduation from an accredited nursing program and a current RN license, along with a current driver's license and CPR proof. Two years of clinical RN experience is preferred, with hospice experience preferred.
