What you'll do

• Conduct in-person physical, cognitive, and functional assessments in members’ homes or community settings
• document findings in the GuidingCare platform and send SOAP notes to the assigned Care Manager
• submit complete documentation within 48 hours and resolve audit findings within two business days
• follow field-safety guidelines, escalate clinical or safety concerns, and comply with mandated reporting, HIPAA, NCQA, and other applicable requirements.
