What you'll do

• Develop comprehensive assessments and care plans or individual support plans

• support beneficiaries in crisis planning
• arrange annual physicals
• coordinate applicable Innovations and TBI waiver services
• convene and consult with multidisciplinary care teams
• maintain clinical documentation and regulatory compliance.
