Case Manager – IOP/PHP

The Case Manager – IOP/PHP works onsite in Lebec, CA, at $20–$25 per hour. What you'll do

• Coordinate care for IOP and PHP clients
• conduct biopsychosocial assessments and assist with treatment planning
• provide case management, referrals, and resource coordination
• facilitate therapeutic groups and individual support sessions
• collaborate with clinical staff and external providers
• support discharge planning and continuity of care
• maintain clinical documentation
• advocate for clients
• monitor progress and barriers
• participate in multidisciplinary meetings.
