DESC’s SAGE outpatient mental health program provides long-term support to adults with severe and persistent mental illness, including help with housing, clinical stability, and independence.

What you'll do

• Provide ongoing case management, outreach, and engagement
• conduct psychosocial assessments and develop goal plans with consumers
• coordinate psychiatric and substance-use treatment and access to essential resources
• advocate for clients and coordinate continuity of care
• conduct local home visits
• provide crisis intervention and risk assessment
• document services and maintain records.

Why this role
