What you'll do

• Support development and expansion of Wraparound Programs
• track family-team progress and outcomes
• liaise with offices and referring agencies
• participate in collaborative planning and community outreach
• contribute to data-driven workflows
• oversee structured daily living activities and safety
• participate in the on-call rotation
• assist with case reviews
• train and mentor staff
• support case assignments and oversee cases
• attend family meetings to guide facilitators and family teams.
