Hospital Liaison / Marketer

What you'll do

• Develop, maintain, and grow referral relationships with physicians, hospital discharge planners, skilled nursing facilities, case managers, assisted living communities, and other healthcare professionals
• promote agency services through in-person visits, presentations, and outreach
• meet monthly referral and admission goals
• collaborate with clinical and intake teams on referrals and admissions
• attend networking and community outreach events
• track referral sources, follow-up activity, and outcomes.
