What you'll do

• Receive and review case referrals and patient information, including home visits, to determine hospice or home care needs
• assign hospice personnel as needed
• review cases through visits, conferences, and records
• guide clinicians
• review diagnoses, prognosis, medications, procedures, and clinical course
• assist with therapeutic goals and plans of care
• attend case conferences
• conduct quarterly record reviews
• assist with hiring, orientation, training, policies, and programs
• participate in community health planning and quality assessment and performance improvement.
