The Hospice Registered Nurse (RN) Case Manager role is onsite, serving patients in homes and facilities across Metro Detroit and surrounding areas. Pay is $82,000–$90,000 per year. Amara Hospice Care’s mission is to provide comprehensive, compassionate, patient-centered care for people with life-limiting illnesses. The case manager assesses patients and families, develops and updates individualized care plans, and coordinates care with healthcare teams. The role also provides end-of-life counseling and emotional support, supervises hospice aides and LPNs, and participates in an on-call rotation.
